These patient instruction forms are designed to assist patients and their caregivers in preparing for their oral surgery.
Tips and Requirements For Your Safety and Comfort
To help all of our patients enjoy timely appointments with us, we ask that patients arrive a minimum of twenty minutes ahead of their stated time to complete new patient forms. If you are able to complete your forms ahead of time, please arrive at our office ten minutes prior to your stated appointment time. New patients must provide the following:
- Completed Patient Registration/Medical History
- Completed Patient Financial Agreement
- Referral slip and/or x-ray from referring or other office
If you do not have internet access to print the forms on our website, you may also obtain them from our office or by requesting they be emailed or mailed to you. In addition, applications are also available for health care financing through Care Credit in our office or via a link on our website. The quick and easy application process for either option can be done online or right in our office. Please do not hesitate to ask any of our front office staff for assistance.
General Anesthesia / I.V. Sedation
- A responsible adult must accompany you to and from the office for the surgery and remain with you for 24 hours afterward.
- Wear short sleeves and loose-fitting clothing to accommodate monitors.
- Do not wear jewelry, mascara, or contact lenses.
- Do not eat or drink anything 6 hours prior to the appointment.
Remember: if you have a history of heart problems, lung problems, stroke, artificial joints, or you take blood thinners or steroids, please be sure to talk with your surgeon at Southwest Oral Surgical Arts (SWOSA) about these conditions before your treatment appointment.
Be sure your surgeon knows of any medications you take prior to surgery and plan to take them as you normally would (with a small amount of water) unless instructed by your doctor otherwise.
